01MVA

Motor Vehicle Accident (MVA) Rehabilitation

If you’ve been injured in a car accident, you are entitled to treatment under Ontario’s Statutory Accident Benefits Schedule — regardless of who was at fault. Our team manages the paperwork and works directly with your insurer so you can focus on recovery. We provide coordinated physiotherapy, chiropractic, massage and psychology for whiplash, soft-tissue injuries, headaches and accident-related trauma, and complete the required treatment plans (OCF forms) on your behalf. You do not need to pay up front — approved treatment is billed directly to your auto insurer.

02IFHP

Interim Federal Health Program (IFHP)

Planet Health Care welcomes patients covered under the Interim Federal Health Program, which provides limited, temporary health coverage for protected persons, refugee claimants and other eligible groups who are not yet covered by provincial health insurance. We’ll confirm your eligibility and covered services before treatment and bill the program directly where applicable, so you can access physiotherapy and related care without cost barriers.

03VAC

Veterans Affairs Canada (VAC)

We are proud to care for Canada’s veterans. Eligible veterans covered by Veterans Affairs Canada can receive physiotherapy, chiropractic, massage and related rehabilitation at our clinic. Bring your VAC authorization or K-number and we’ll bill VAC directly for approved services — no out-of-pocket cost for covered treatment. Our team will help coordinate any approvals your treatment requires.

04FAF

FAF Forms & Functional Assessments

Need a Functional Abilities Form (FAF) or similar return-to-work or insurer documentation completed? Our registered practitioners perform the assessment and complete FAF and related forms accurately and promptly, so your claim, employer or insurer has what they need. Book an assessment and let us know which form you require when you call.
